    Public Health Nurse Intervention for Depression Screening during Sexually Transmitted Disease Risk Assessments

    The United States Preventive Services Task Force (2016) recommends screening adolescents and adults for depression. Depression screening is important during sexually transmitted disease (STD) risk assessments due to the association between depression and STD risk behavior. STD Enhanced Role Registered Nurses (ERRNs) are ideally positioned to incorporate depression screening into STD risk assessments. Purposes: 1. Expand the STD ERRNs' education to include an understanding of the association of depression with STD risk factors, be able to identify those at high risk for depression, and give them an evidence-based tool to screen for depression, thus enhancing the STD ERRNs' comfort and self-efficacy to screen for depression to facilitate evidence-based practice change in the clinical setting 2. Provide the STD ERRNs with a temporary policy addendum and depression screening guidelines to support them as they perform depression screens in the STD clinic Methods: A Needs Assessment Survey was given to STD ERRNs to determine knowledge and practice gaps. A pilot educational course was provided to one local health department (LHD). A temporary policy addendum was created and granted to eliminate the policy barrier. The course, pretest, and posttest were sent to all remaining STD ERRNs. Two follow-up surveys were sent to determine depression screening goal attainment, barriers, and supportive factors. Results: The Needs Assessment Survey found a low percentage of STD ERRNs screened for depression due to time and policy barriers. Pilot LHD feedback included changes to the screening tool and development of an implementation timeline. The course showed an improvement in comfort levels with administering depression screens and reported improved knowledge of depression and risk for STDs. The follow-up surveys showed that the majority of STD ERRNs were able to meet or exceed their depression screening goals, although some STD ERRNs experienced a time barrier related to the referral process. Limitations: Limitations included the reliability/validity of the pretest and posttest test questions, self-report nature of the survey questions, and small sample size (Phase 1 n = 63; Phase 2 n = 35; Phase 3 1st follow-up n = 14, Phase 3 2nd follow-up n = 17)

    Dalam setiap bahasa terdapat kata-kata yang dianggap tidak sopan untuk dikatakan kepada lawan bicara. Kata-kata tersebut mengacu kepada kata-kata yang mengandung makian dan sumpah serapah, serta memiliki konotasi kasar dan tabu untuk diucapkan dalam situasi formal. Dalam era globalisasi ini, banyak remaja khususnya remaja Sekolah menengah Pertama yang menggunakan umpatan dalam komunikasi mereka sehari-hari. Oleh karena itu, penulis tergugah untuk menganalisis fenomena penggunaan kata umpatan oleh siswa SMPN 1 di wilayah Kotabaru Karawang. Tujuan dari penelitian ini adalah untuk menunjukkan dan menganalisis penggunaan kata umpatan yang digunakan oleh siswa SMP mengacu kepada teori Sosiolinguistik yang dikemukakan oleh Janet Holmes. Kata-kata umpatan yang diproduksi oleh siswa merupakan penelitian yang bersifat deskriptif dengan pendekatan kualitatif. Penulis mengambil data dengan teknik purposive sampling sehingga penulis mendapat data sebanyak 25 kata umpatan yang dalam pengumpulan datanya dibagi menjadi 2 tahap yaitu wawancara dan observasi. Penulis juga menggunakan teknik Simak Bebas Libat Cakap dan teknik catat ketika mengobservasi tuturan umpatan yang digunakan oleh siswa. Dalam mewawancarai siswa, penulis menggunakan teknik rekam untuk menjaga keaslian data. Dari hasil pengumpulan data, penulis mendapatkan 16 tuturan umpatan yang diperoleh dengan cara mewawancarai siswa dan 9 tuturan umpatan yang diperoleh dengan cara observasi di sekolah. Sebagian besar kata – kata umpatan dituturkan oleh siswa laki – laki. Namun ada beberapa tuturan umpatan yang diproduksi oleh siswa perempuan ketika mengumpat dengan teman sebayanya. Dari hasil observasi, penulis menemukan kata – kata umpatan yang digunakan siswa kepada temannya hanya pada latar informal. Walaupun ada seorang murid yang mengaku bahwa dirinya pernah mengumpat pada saat terdapat guru di dalam kelas, hal itu tidak lebih dari sekedar lelucon belaka. Secara garis besar, topik ketika siswa mengumpat kepada temannya hanya sebagai bahan lelucon. Meskipun ada beberapa tuturan umpatan yang mempunyai topik kemarahan, sebagian besar umpatan yang digunakan siswa mempunyai fungsi ekspresif

    Experimental Test of Connector Rotation during DNA Packaging into Bacteriophage ϕ29 Capsids

    The bacteriophage ϕ29 generates large forces to compact its double-stranded DNA genome into a protein capsid by means of a portal motor complex. Several mechanical models for the generation of these high forces by the motor complex predict coupling of DNA translocation to rotation of the head-tail connector dodecamer. Putative connector rotation is investigated here by combining the methods of single-molecule force spectroscopy with polarization-sensitive single-molecule fluorescence. In our experiment, we observe motor function in several packaging complexes in parallel using video microscopy of bead position in a magnetic trap. At the same time, we follow the orientation of single fluorophores attached to the portal motor connector. From our data, we can exclude connector rotation with greater than 99% probability and therefore answer a long-standing mechanistic question

    The international generalisability of evidence for health policy: a cross country comparison of medication adherence following policy change

    Copayments for prescriptions may increase morbidity and mortality via reductions in adherence to medications. Relevant data can inform policy to minimise such unintended effects. We explored the generalisability of evidence for copayments by comparing two international copayment polices, one in Massachusetts and one in Ireland, to assess whether effects on medication adherence were comparable. We used national prescription data for public health insurance programmes in Ireland and Medicaid data in the U.S. New users of oral anti-hypertensive, anti-hyperlipidaemic and diabetic drugs were included (total n = 14,259 in U.S. and n = 43,843 in Ireland). We examined changes in adherence in intervention and comparator groups in each setting using segmented linear regression with generalised estimating equations. In Massachusetts, a gradual decrease in adherence to anti-hypertensive medications of −1% per month following the policy occurred. In contrast, the response in Ireland was confined to a −2.9% decrease in adherence immediately following the policy, with no further decrease over the 8 month follow-up. Reductions in adherence to oral diabetes drugs were larger in the U.S. group in comparison to the Irish group. No difference in adherence changes between the two settings for anti-hyperlipidaemic drugs occurred. Evidence on cost-sharing for prescription medicines is not ‘one size fits all’. Time since policy implementation and structural differences between health systems may influence the differential impact of copayment policies in international settings

    The Mother Centriole Plays an Instructive Role in Defining Cell Geometry

    Centriole positioning is a key step in establishment and propagation of cell geometry, but the mechanism of this positioning is unknown. The ability of pre-existing centrioles to induce formation of new centrioles at a defined angle relative to themselves suggests they may have the capacity to transmit spatial information to their daughters. Using three-dimensional computer-aided analysis of cell morphology in Chlamydomonas, we identify six genes required for centriole positioning relative to overall cell polarity, four of which have known sequences. We show that the distal portion of the centriole is critical for positioning, and that the centriole positions the nucleus rather than vice versa. We obtain evidence that the daughter centriole is unable to respond to normal positioning cues and relies on the mother for positional information. Our results represent a clear example of “cytotaxis” as defined by Sonneborn, and suggest that centrioles can play a key function in propagation of cellular geometry from one generation to the next. The genes documented here that are required for proper centriole positioning may represent a new class of ciliary disease genes, defects in which would be expected to cause disorganized ciliary position and impaired function

    MultiCellDS: a community-developed standard for curating microenvironment-dependent multicellular data

    Exchanging and understanding scientific data and their context represents a significant barrier to advancing research, especially with respect to information siloing. Maintaining information provenance and providing data curation and quality control help overcome common concerns and barriers to the effective sharing of scientific data. To address these problems in and the unique challenges of multicellular systems, we assembled a panel composed of investigators from several disciplines to create the MultiCellular Data Standard (MultiCellDS) with a use-case driven development process. The standard includes (1) digital cell lines, which are analogous to traditional biological cell lines, to record metadata, cellular microenvironment, and cellular phenotype variables of a biological cell line, (2) digital snapshots to consistently record simulation, experimental, and clinical data for multicellular systems, and (3) collections that can logically group digital cell lines and snapshots. We have created a MultiCellular DataBase (MultiCellDB) to store digital snapshots and the 200+ digital cell lines we have generated. MultiCellDS, by having a fixed standard, enables discoverability, extensibility, maintainability, searchability, and sustainability of data, creating biological applicability and clinical utility that permits us to identify upcoming challenges to uplift biology and strategies and therapies for improving human health

    MultiCellDS: a standard and a community for sharing multicellular data

    Cell biology is increasingly focused on cellular heterogeneity and multicellular systems. To make the fullest use of experimental, clinical, and computational efforts, we need standardized data formats, community-curated "public data libraries", and tools to combine and analyze shared data. To address these needs, our multidisciplinary community created MultiCellDS (MultiCellular Data Standard): an extensible standard, a library of digital cell lines and tissue snapshots, and support software. With the help of experimentalists, clinicians, modelers, and data and library scientists, we can grow this seed into a community-owned ecosystem of shared data and tools, to the benefit of basic science, engineering, and human health
